2010-2011 Men's Ball Hockey Registration

• COST: This year we are starting something new. We are going to have an early bird special. To receive the early bird special of $75, you must register and pay before Sept 22/10. After Sept 22, the cost goes up to $85 and will be due by Oct 6/10 (the first night of the season). The fee includes all games & a team t-shirt.
• STICK: Every player needs a league stick. They can be purchased for $15 or $25 through this registration form if you don't already own one.  What's the difference? The $25 is a superior stick with less flex, while $15 sticks are older and have more flex to them. (There is a limited quantity of $15 sticks and they will be sold on a first-come, first-serve basis.)
• HOW DO I SUBMIT MY FEE: Your league payment can paid in one of 3 ways: (1) In person at Stoney Creek on Wednesday, September 8, 2010 from 7pm-8pm; (2) By mailing a cheque to: Men's Ball Hockey League, Stoney Creek Baptist Church, 2225 Highbury Ave North, London, ON, N5X4A4; Or, (3) Your payment can be dropped off at the church office any time during office hours (M-F, 9am-3pm).
  
• GAMES: Games are played Wednesday nights at 7pm, 8pm, and 9pm at Stoney Creek Baptist Church in London, Ontario. Games begin in October 2010 and end at the beginning of March 2011.
  
• REQUIRED EQUIPMENT: Soccer shin-pads & socks, eye goggles, & non-marking running shoes (all of which must be purchased on your own prior to the first game.)
  
• AGE: Players must be men, 20 years of age or older.
• PLAYING ON A FRIEND'S TEAM: On the registration form below, you'll see the box: "Enter names of players you'd like to play with." Type in the names of players you'd like to be on the same team as (and make sure they type in your name as well) -- and we will do our best to accommodate your request. We cannot, however, offer any guarantees.
